A company based in Suffolk has been prosecuted after dangerous conditions were found in its factory near Bury St Edmunds.
The Health and Safety Executive (HSE) carried out an investigation on the company, Suffolk Mushrooms Limited, for a number of safety failings.
The gates on the work platforms used by employees to reach the highest mushroom beds, were propped open and a heavy goods vehicle regularly reversed 100m between the two growing sheds where people might be walking.

Construction company and director fined for safety failings |

A construction company and its director have been prosecuted after carrying out unsafe demolition and construction work at a house in Surrey.
The Edgware-based firm, Laxmi Developments Ltd and its director, Vijay Madhaparia, failed to comply with a Prohibition Notice (PN), did not provide adequate safety protection for employees and failed to carry out an asbestos survey on a property before demolition.
The Health and Safety Executive (HSE) investigated the building site at Mellow Close in Banstaed, where the inspector was so shocked by such poor standards that he served a PN and three Improvement Notices on the firm.

Landlord prosecuted after putting tenant at risk of gas leak |

A landlord in Wigan has been fined after he put the life of his tenant at risk by falsifying a gas safety document.
Terence McBride's tenant had contacted authorities after suspecting that there was a carbon monoxide leak in the house, which she rented on Bell Lane in Pemberton in May 2011. A National Grid engineer visited the property and sealed off the gas supply after finding a leak.
Trafford Magistrates' Court heard that Mr McBride had arranged a gas safety check at the house through his letting agents in April 2009, which expired a year later.

Construction firmed fined after putting employees at risk |

The director of a construction firm in Cardiff has been fined after he failed to take proper measures in the safety of his employees working at height.
Paul Wright, who was trading as Cartwright Construction, had failed to put any safety measures in place to protect his employees while they were working at a building site off Conway Road, Pontcanna, Cardiff.
During a visit to the site, a Health and Safety (HSE) Inspector observed two employees standing on the outer edge of a partially-completed first storey structure.
